Chairman of the Board of Directors CSMS-IPA
David Thompson, M.D., is the Chairman of the Board of Directors of the CSMS-IPA.
Dr. Thompson provides the overall strategic leadership and clinical direction for the CSMS-IPA and the physician lead Board of Directors. Dr. Thompson is also a full-time primary care physician with extensive experience in value-based care programs.

President and Chief Executive Officer
Christopher M. Cannon, MSN, MPH, MBA, is the President and Chief Executive Officer of the CSMS-IPA. Christopher is a senior healthcare leader with advanced degrees in nursing, public health and business administration. His experience includes inpatient and ambulatory services administration in large academic medical centers, value-based healthcare insurance programs, physician practice development and operations, and best-practice clinical consulting with both private and governmental organizations.

Senior Clinical Manager
Kara Moonan, DNP, APRN, PNP-BC is our Physician Network Clinical Manager. Kara is a dedicated provider with experience working across multiple healthcare settings including FQHCs, homecare, educational systems, residential settings, as well as primary care practices. Kara has worked as a primary care provider for many years and understands the challenges faced by clinicians to provide high quality and comprehensive care to their patients. Kara in addition, to her clinical role has also supported many organizations in pursuing best practices, improved workflows, and expanding integrated healthcare sservice.
Physician Network Operations Efficiency Manager
Martin is an experienced registered nurse and healthcare leader with a strong background in managed care, network operations, and value-based care performance. He has extensive experience collaborating with physicians, payors, and provider organizations to improve care transitions, and support quality outcomes across acute, post-acute, and home health settings. Known for a data-driven and collaborative approach, Martin focuses on aligning clinical best practices with operational performance to support high-quality, cost-effective patient care.

Physician Network Clinical Specialist
Jeanette Burke, MSN, RN, serves as a Physician Network Clinical Specialist. She brings many years of comprehensive clinical experience across acute care, post-acute settings, case management, and managed care. Jeanette has led initiatives in data reporting and auditing and has developed processes and workflow enhancements to optimize clinical performance. Her diverse background enables Jeanette to approach challenges with creativity and a multifaceted perspective, consistently identifying effective solutions.

Physician Network Clinical Specialist
Hayley Vyas RN, BSN is a Physician Network Clinical Specialist. She is a dedicated healthcare professional with over 12 years of diverse clinical experience across multiple specialty areas. Hayley most recently worked in Managed Care serving as a Clinical Case Manager and Reviewer for post-acute facilities to ensure appropriate levels of care and alignment with medical-necessity criteria. Her ability to assess clinical information and determine medical necessity reflects both her clinical expertise and commitment to value based care delivery.

Carla is a Physician Network Clinical Specialist LPN, with extensive clinical nursing experience ranging from surgical & specialty to assisted living. Carla brings an expansive knowledge of patient care, clinical processes, and healthcare systems. Her background has equipped her with critical thinking and compassion.

Physician Network Coordinator
Cindy is the Physician Network Coordinator for the CSMS-IPA administratively supporting healthcare over her career including sales and marketing. Cindy support to IPA physicians and their practices in key areas related to documentation and program requirements. She also supports physician practice in network support for payers exceeding quality outcome

Physician Network Operations & Data Coordinator
Evens is the CSMS-IPA as the Database Coordinator, a motivated and organized IT professional with experience in analyzing, designing, and implementing business process improvements. He has an Associate of Science degree in Information Technology and a Certification in Web development. Evens provides impeccable customer service and through his expertise of IT functions and processes delivers timely support to the IPA physicians networks.
