Neysa, new to the IPA in January 2013, has extensive experience in nonprofit management and grant administration. She has overseen the development and implementation of federal and state programs related to keeping older adults and individuals with disabilities in the community through support services including care management, respite services and care transitions. Neysa is committed to person-centered and coordinated care.

Tamara J. King, RN MSW CCM

Tamara has contributed to the social services and health care market for the past 16 years. She brings with her an extensive background in assessment and care planning, care coordination, care transitions, disease management, and case management. In her experience, she realized the importance of patient-engagement as it relates to the success of positive outcomes. With an education in both the Social Work and Nursing fields, she hopes to bring a unique approach to the IPA, where value-based and clinical initiatives foster strong business-medicine partnerships.

Fiorina Treloar, LPN
Clinical Specialist

Fiorina recently joined CSMS-IPA in October 2021 as the LPN Clinical Specialist. She brings over 13 years of diverse healthcare knowledge and expertise including several years employed as a Certified Pharmacy Technician and has worked in specialty practice and primary care settings. She is motivated to drive value-based care initiatives and to continue to collaborate with independent providers to achieve the goal of patient success and improved outcomes.

Kathy Carlson
Physician Network Specialist

Kathy recently joined the CSMS-IPA staff as Physician Network Specialist. In her role she will support all functions of the CSMS-IPA clinical team. Her primary responsibility is to engage physician offices around patient management with emphasis on clinical initiatives and best practices. She will also provide health plan payer and third party vendor coordination. Kathy’s career includes positions in practice management, patient outreach coordination and management of the hospitalist program at a community hospital. In these roles she served as the patient liaison, conducted quality auditing and lead the implementation of a Health Information Exchange for a community hospital system and it’s community providers. Her experiences align with helping our members achieve the Triple Aim.

Pattie Mastriano
Physician Network Specialist

Pattie has thirty four years combined service with the CSMS-IPA and the Connecticut State Medical Society (CSMS). Pattie’s knowledge of the organizations and physician network lends to her ability to be a resource for the IPA physicians and liaison between the varieties of organizations in the health care market place. Pattie enjoys applying her organizational skills to support CSMS-IPA physicians and management in planning various organizational events in a statewide capacity. In addition, Pattie is responsible for corporate office functions including accounts receivable and payable and legal document coordination.

Cindy Agosto

Cindy has been in the administrative field of health care for sixteen years. For three years she was in the marketing and sales area. Cindy has been with the CSMS-IPA for the past thirteen years, where she provides support to IPA physicians and their practices in key areas related to documentation and program requirements. She also triages calls to be sure they are directed to the appropriate department and that responses are made in a timely manner.

Evens Borno
Database Coordinator

Evens is a motivated and organized IT professional with experience in analyzing, designing, and implementing business process improvements. He has an Associate of Science degree in Information Technology and a Certification in Web development. Evens is committed to providing impeccable customer service and through his acquired expertise of IT functions and processes delivers timely support to the analysis team. During his tenure with CSMS-IPA as the Database Coordinator, he has developed his knowledge of healthcare delivery and utilizes that information as a frame of reference for all of his work.
